Immunohistochemical localization of adducts can be accomplished using antiserum developed and characterized for quantitative immunoassays. The 3-(cystein-S-yl)acetaminophen protein adduct is associated with the toxicity of the prototype hepatotoxin, acetaminophen. Immunohistochemical localization of this adduct is described to illustrate the technique as an adjunct to other methods in the assessment of toxicity. This method provides direct correlation between presence of adduct and morphologic evidence of cell injury. Microwave irradiation was pioneered as a fixation method to simultaneously preserve tissue structure and adduct antigenicity. Many chemicals that are active as toxicants or carcinogens have electrophilic properties or are metabolically converted to electrophiles which react with nucleophilic centers in nucleic acids and proteins to form covalent adducts. Covalent binding of a reactive metabolite to essential cell constituents is frequently a critical event that leads to toxicity. The complex formed by reactive metabolite binding to cell macromolecules (adduct) constitutes a novel molecular species in the organism, and it may contain unique antigenic determinants or "neoantigens". Immunological assays that recognize these neoantigens can be used to study the role of the macromolecular adduct in toxicity. Once an important neoantigen is known, antisera raised against an immunogen that contains the critical epitope may be the basis of a variety of immunoassays. Since the parent compound is usually a hapten which, by itself, is too small to be an immunogen, appropriate immunogenic conjugates must be isolated or synthesized. Antisera raised against macromolecular conjugates of the parent compound or a structurally related metabolite may recognize 1) the parent compound 2) the modified macromolecule, or 3) a unique adduct. T h u s , t h e u s e of h i g h l y c h a r a c t e r ized antisera is essential to meaningful interpretation. I n m a n y c a s e s , a n t i b o d i e s developed t o d e t e c t a n d q u a n t i f y t h e s e a n t i g e n i c a d d u c t s (and o t h e r a n t i g e n i c t a r g e t molecules) are a l s o s u i t a b l e for u s e i n p a r a l l e l i m m u n o h i s t o c h e m i c a l s t u d i e s . T h e s e s t u d i e s c a n pro v i d e b o t h s p a t i a l a n d t e m p o r a l c o r r e l a t i o n of a d d u c t l o c a l i z a t i o n w i t h c e l l i n j u r y a n d t h u s p r o v i d e i n f o r m a t i o n o n p a t h o g e n e s i s t h a t c a n n o t be ob t a i n e d b y o t h e r m e a n s . T h e i n t e g r i t y of i n f o r m a t i o n g a i n e d f r o m i m m u n o h i s t o c h e m i c a l l o c a l i z a t i o n of a n t i g e n s i s c r i t i c a l l y d e p e n d e n t o n k n o w l e d g e of t h e a n t i g e n i c d e t e r m i n a n t s t h a t t h e p r i m a r y a n t i b o d y w i l l bind. A s p a r t of t h e l o c a l i z a t i o n p r o c e d u r e , t h e a n t i g e n - b o u n d p r i m a r y a n t i b o d y i s d e t e c t e d b y a s y s t e m t h a t p e r m i t s d i r e c t o b s e r v a t i o n of t h e s i t e of a n t i b o d y b i n d i n g . A n u m b e r of d e t e c t i o n s y s t e m s are a v a i l a b l e t o enhance s e n s i t i v i t y a n d to visualize p r i m a r y antibody b o u n d i n s i t u to t a r g e t a n t i g e n . M o s t of t h e m u s e e i t h e r a r a d i o l a b e l e d s e c o n d a n t i b o d y a n d v i s u a l i z a t i o n b y a u t o r a d i o g r a p h y , or a n e n z y m e , fluorescent, or elec t r o n d e n s e p r o b e c o u p l e d t o a l i n k i n g s e c o n d a n t i b o d y , p r o t e i n A , or a v i d i n - b i o t i n a m p l i f i c a t i o n s y s t e m (1,2). I m m u n o h i s t o c h e m i s t r y p r o v i d e s s t r u c t u r a l s p e c i f i c i t y t h a t i s ab s e n t i n a u t o r a d i o g r a p h y of r a d i o l a b e l e d c o m p o u n d s a n d f u r t h e r affords t h e o p p o r t u n i t y t o s t u d y h u m a n t i s s u e w i t h o u t h a v i n g f i r s t to a d m i n i s t e r a r a d i o a c t i v e c o m p o u n d . I n a d d i t i o n , i m m u n o h i s t o c h e m i c a l tech n i q u e s h a v e t h e a d v a n t a g e t h a t m a n y a n t i g e n s are p r e s e r v e d b y s o l u t i o n s u s e d for h i s t o l o g i c f i x a t i o n , e.g., f o r m a l d e h y d e . T h u s , t i s s u e s p r e s e r v e d for o t h e r p u r p o s e s c a n be s t u d i e d r e t r o s p e c t i v e l y to c o r r e l a t e h i s t o p a t h o l o g y , a d d u c t l o c a l i z a t i o n a n d o t h e r i n d i c e s of i n j u r y . I n p r i n c i p a l , t h e m e t h o d l e n d s i t s e l f w e l l to i d e n t i f i c a t i o n of b i o m a r k e r s of t o x i c a n t e x p o s u r e i n t h e c o n t e x t of " m o l e c u l a r e p i d e m i o l o g y " . A l t h o u g h a v a r i e t y of f i x a t i o n t e c h n i q u e s are a v a i l a b l e , n o s i n g l e s y s t e m p r o v i d e s o p t i m a l p r e s e r v a t i o n of m o r p h o l o g y of a l l t i s s u e s a n d a l l t a r g e t a n t i g e n s s i m u l t a n e o u s l y . T h u s t h e r e i s a s p e c t r u m of effectiveness i n a c h i e v i n g g o o d p r e s e r v a t i o n of b o t h s t r u c t u r e a n d a n t i g e n i c i t y , i n c l u d i n g s e v e r a l h i g h l y s a t i s f a c t o r y t e c h n i q u e s (3-6). F o r p r o s p e c t i v e s t u d i e s , we h a v e s u c c e s s f u l l y p i o n e e r e d the s i m u l t a n e o u s p r e s e r v a t i o n of a d d u c t a n t i g e n i c i t y a n d t i s s u e s t r u c t u r e u s i n g f i x a t i o n b y m i c r o w a v e i r r a d i a t i o n (7). M i c r o w a v e i r r a d i a t i o n h a d been d e m o n s t r a t e d t o p r o v i d e s u p e r i o r p r e s e r v a t i o n of a n t i g e n s u s e d i n d i a g n o s t i c p a t h o l o g y . F o r 22 of 23 a n t i g e n s i n n i n e t i s s u e s i n c o m p a r i s o n w i t h formaldehyde, it yielded i m m u n o s t a i n i n g that was more intense and m o r e e x t e n s i v e w h i l e p r o v i d i n g e x c e l l e n t p r e s e r v a t i o n of c y t o m o r p h o l o g i c d e t a i l (8). B y c a u s i n g e x c i t a t i o n of w a t e r m o l e c u l e s , t h e m i c r o w a v e en e r g y effects c o n t r o l l e d h e a t i n g a n d c o a g u l a t i v e s t a b i l i z a t i o n of p r o t e i n .

I m m u n o l o g i c t e c h n i q u e s were u s e d t o s t u d y a c e t a m i n o p h e n t o x i c i t y i n m i c e t o e l u c i d a t e m e c h a n i s m s of l i v e r t o x i c i t y . T h e h e p a t o t o x i c i t y of acetaminophen is mediated b y a reactive metabolite, N-acetyl-p-benzoquinone i m i n e ( N A P Q I ) . T h e metabolite b i n d s to protein as 3 - ( c y s t e i n - S - y l ) a c e t a m i n o p h e n ( 3 - C y s - A ) a n d t h e a m o u n t of b i n d i n g corre lates w i t h t o x i c i t y . T h i s covalent b i n d i n g , a n d i n p a r t i c u l a r the 3 - C y s - A a d d u c t , i s t h e m o s t r e l i a b l e b i o m a r k e r of a c e t a m i n o p h e n t o x i c i t y (9-12). P r e v i o u s r e p o r t s d e s c r i b e d : a n E L I S A specific for 3 - C y s - A p r o t e i n a d d u c t s t h a t does n o t r e l y o n r a d i o l a b e l e d s a m p l e s (.13); c h a r a c t e r i z a t i o n of t h e r e l e v a n t epitope (14); a n d q u a n t i f i c a t i o n of 3 - C y s - A a d d u c t s i n l i v e r a n d s e r u m as a f u n c t i o n of h e p a t o t o x i c i t y (9). A p p r o a c h . M a l e B 6 C 3 F 1 m i c e were f a s t e d o v e r n i g h t a n d d o s e d w i t h s a l i n e (control) or a c e t a m i n o p h e n (400 m g / k g ) i n t r a p e r i t o n e a l ^ t h e fol l o w i n g m o r n i n g . T o t a l h e p a t i c g l u t a t h i o n e ( G S H ) levels were d e t e r m i n e d , a c c o r d i n g t o t h e m e t h o d of T i e t z e (16), as a n i n d i c a t o r of t h e degree t o w h i c h r e a c t i v e m e t a b o l i t e h a d d e p l e t e d t h i s c o n s t i t u t i v e d e t o x i f i c a t i o n m e c h a n i s m (11). T h e p o l y c l o n a l a n t i s e r u m u s e d for t h e s e s t u d i e s w a s r a i s e d i n rab bits i m m u n i z e d w i t h 3-(N-acetylcystein-S-yl)acetaminophen-keyhole lim p e t h e m o c y a n i n a n d h a s s p e c i f i c i t y for a n t i g e n i c d e t e r m i n a n t s w h i c h are n o t f o u n d o n t h e p a r e n t c o m p o u n d or o n h o s t p r o t e i n , b u t are f o u n d o n a d d u c t s i n w h i c h a c e t a m i n o p h e n i s b o u n d v i a c a r b o n 3 t o t h e s u l f u r of c y s t e i n e i n p r o t e i n s (13,14, R o b e r t s et a l . , t h i s v o l u m e ) . C o m p a r i s o n of F i x a t i o n Techniques. F o r microwave fixation, fresh tis sues were t r i m m e d to a p p r o x i m a t e l y 2 m m t h i c k n e s s , p l a c e d i n p l a s t i c c a s s e t t e s , a n d h e l d i n ice c o l d s a l i n e (30-120 m i n u t e s ) u n t i l i r r a d i a t i o n i n a 7 0 0 - w a t t o v e n set to h o l d at 6 0 ° C for 2 m i n u t e s (7). M i c r o w a v e - f i x e d t i s s u e s were s t o r e d o v e r n i g h t i n 7 0 % e t h a n o l at r o o m t e m p e r a t u r e , proc essed r o u t i n e l y a n d then embedded i n paraffin, sectioned, deparaffinized, a n d r e h y d r a t e d u s i n g s t a n d a r d t e c h n i q u e s . O t h e r pieces of l i v e r were s i m i l a r l y t r i m m e d , then fixed b y i m m e r s i o n i n either 3.7% formaldehyde i n n e u t r a l p h o s p h a t e buffer (48 h o u r s ) , 2 % g l u t a r a l d e h y d e i n p h o s p h a t e buffer (2 h o u r s ) , B o u i n ' s s o l u t i o n (24 h o u r s ) , or B - 5 s o l u t i o n (4 h o u r s ) . I n addition, the data support the concept t h a t N A P Q I r e a c t s w i t h p r o t e i n a t t h e s i t e of i t s f o r m a t i o n a n d does n o t diffuse f r o m c e n t r i l o b u l a r h e p a t o c y t e s t o o t h e r c e l l s s u c h as p e r i p o r t a l h e p a t o c y t e s . T h i s s y s t e m was also used to demonstrate 3 - C y s - A adducts i n the m o u s e k i d n e y a n d l u n g . T h r e e h o u r s after 4 0 0 m g / k g a c e t a m i n o p h e n , adduct was present i n m o s t p r o x i m a l a n d some d i s t a l convoluted tubules i n t h e k i d n e y ( F i g u r e 7). I n t h e l u n g , a d d u c t s were r e s t r i c t e d t o e p i t h e l i a l c e l l s l i n i n g t h e b r o n c h i a n d b r o n c h i o l e s , s i t e s p r e v i o u s l y r e p o r t e d t o be t h e t a r g e t of a c e t a m i n o p h e n - i n d u c e d n e c r o s i s (20) a n d t h e l o c a t i o n of t h e e t h a n o l - i n d u c i b l e P - 4 5 0 (19) ( F i g u r e 8). Conclusion U s i n g a d r u g - p r o t e i n a d d u c t as a b i o m a r k e r of c e l l i n j u r y , we h a v e s h o w n t h a t i m m u n o h i s t o c h e m i s t r y c a n be a p o t e n t t e c h n i q u e t o c o m p l e m e n t other i m m u n o c h e m i c a l analyses to reveal i n s i t u the molecular i n s u l t associated w i t h pathologic change at the subcellular level. W e l l c h a r a c t e r i z e d a n t i b o d y i s e s s e n t i a l , as are w e l l p r e s e r v e d t i s s u e s t r u c t u r e s a n d a d d u c t a n t i g e n i c i t y . C o r r o b o r a t i v e evidence afforded b y o t h e r t e c h n i q u e s , b o t h q u a n t i t a t i v e a n d t e m p o r a l , p r o v i d e s a m p l e v a l i d a t i o n of the localization procedure. I m m u n o h i s t o c h e m i s t r y is subjectively semi q u a n t i t a t i v e . C a r e f u l a t t e n t i o n t o p r o c e d u r a l d e t a i l s s u c h as s e c t i o n t h i c k n e s s , i n c u b a t i o n p e r i o d s , a n d s t a n d a r d i z e d m e t h o d s i m p r o v e repro ducibility and quantitation. A u t o m a t e d image analysis systems u s i n g d e n s i t o m e t r y or p h o t o m e t r y c a n provide further enhancement. These m e t h o d s c a n be e m p l o y e d u l t r a s t r u c t u r a l l y as w e l l , to p r o v i d e g r e a t e r d e t a i l r e g a r d i n g t h e l o c a l i z a t i o n of a n t i g e n i c m a r k e r s . poses. For example, they can be used after the fact to identify possible toxic agents in human tissues. Immunohistochemical surveys provide a potential approach to environmental monitoring in which sentinel animals or plants could be collected and evaluated for presence of adducts that were biomarkers of exposure. Localization of a specific adduct in tissue provides a visual fixed point of reference from which further questions may be posed regarding other correlates of injury: altered structure or function and quantification of adduct. In the case of acetaminophen adducts in the liver, we showed that morphologic evidence of cell injury (histology), coincided consistently in time and location with quantity of localized adduct. Functional measurements revealed that clinical evidence of hepatic dysfunction also were proportional to histologic evidence of localized adduct and of injury. The concordance between adduct visualized in tissue sections and adduct measured in liver homogenates by quantitative immunoassay was temporally consistent as the adduct accumulated and also later as the adduct leaked from damaged cells and appeared in serum (7). Other correlations that strengthened the postulated mechanism of acetaminophen hepatotoxicity were the depletion of hepatic GSH antecedent to cell injury, and the coincident distribution of localized adduct, tissue injury, and ethanol-inducible P-450.
